Your First Appointment

You’ll receive an email confirmation from us with a copy of our new patient paperwork after you schedule your initial appointment. Our paperwork package includes an extensive health history questionnaire, informed consent, office policies, and HIPPA information forms. Please complete the health history section and read and sign the informed consent, office policies, and notice of privacy practices. Bring these with you to your appointment or scan and send them to us beforehand. If you cannot access forms electronically let us know and be prepared to arrive 20 minutes early to fill out your forms in our waiting area before your scheduled appointment time.

  • Our health history questionnaire is 5 pages because we want to know everything about your health that we can. Traditional Chinese Medicine is a complete diagnostic medical system with a holistic approach. This means that if you are coming in for help managing acid reflux it is helpful to know if you also experience headaches, have knee pain, and trouble sleeping. These symptoms may seem unrelated but it all helps to identify the correct diagnosis and treatment for you from a TCM perspective. The more information you are willing to share the better, especially for those interested in utilizing herbal medicine as part of a comprehensive treatment plan.

  • WellSpace Acupuncture is nestled inside the Wellness Seed Collective and our office is located on the lower level. Those coming in the front of the building can take the stairs at the end of the hallway down to the lower level. To avoid the stairs, feel free to come in the lower level entrance located in the back of the building. Ample parking is available on both the front and back of the building. Once inside have a seat in the waiting area; Jaimie will greet you and bring you back to the treatment space at your appointment time.

  • Yes, your first appointment will last 90 minutes; we’ll spend 30-45 minutes going over your health history, your goals, and treatment plan, leaving enough time for full therapy session.
